Neighbourhood Health System - Warrington East Neighbourhood

What is the Neighbourhood System?

The Neighbourhood System brings together local GP practices, community healthcare teams, social care, mental health services, public health, and voluntary and charity organisations into joined-up Integrated Neighbourhood Teams (INTs).

Rather than patients having to navigate fragmented services separately, local providers will work as one coordinated team focused on the specific health needs of our local East Warrington community.

How does this fit into wider NHS structures?

  • Integrated Care Systems (ICSs): NHS Cheshire and Merseyside continues to serve as our overarching statutory Integrated Care Board (ICB), overseeing strategic planning and resource allocation across the wider region.

  • Neighbourhood Footprints (East Warrington): Day-to-day healthcare delivery is moving closer to home. Local decision-making and service delivery are devolving down to "Place" and "Neighbourhood" levels.

  • Primary Care Networks (PCNs): Warrington Central East PCN remains a core building block inside the East Warrington Neighbourhood, ensuring your local GP practice works hand-in-hand with local community nurses, therapists, social prescribers, and council services.

What does this mean for our patients?

  1. More Joined-Up Care: If you have complex health needs, a long-term condition, or frailty, your GP, community nurse, social care worker, and local specialists will share information and coordinate your care together under a single plan.

  2. Care Closer to Home: The national strategy aims to expand local community diagnostic tools, urgent community response teams, and virtual wards to help treat more people safely at home or in local community settings, reducing unnecessary hospital visits.

  3. Proactive & Preventive Health: By joining forces with local public health, family hubs, and voluntary services in East Warrington, we can focus on keeping people healthy, active, and independent for longer.
